Medical Plan
(Arnett HMO)
$500 individual deductible or $1,000 family deductible per calendar
year.
$20 co-pay for office services by primary care physician.
Other physician services will be paid at 90% after deductible.
30 day waiting period for enrollment.
Cost to full time employee - $21.13 per pay period (every 2 weeks).
Full time employee plus one dependent - $44.38 per pay period (every
2 weeks).
Full time employee with family - $65.61 per pay period (every 2 weeks).
Life Insurance Plan
Equal To 2 times employee's annual gross income (not to exceed $350,000).
Additional 2 times for accidental death or dismemberment.
At the attained age of 70, benefit is reduced by 35%.
At the attained age of 75, benefit is reduced by 60%.
60 day waiting period for enrollment.
Accelerated benefits rider available for insured employees only.
Disability Insurance
Benefits payable on 1st day for disability due to injury.
Benefits payable on 8th day for disability due to sickness.
Reimburses at 50% of pre disability earnings.
60 day waiting period for enrollment.
Annual Leave
15 days accrued annually with less than 2 years of service.
20 days accrued annually with 2 years but less than 7 years of service.
25 days accrued annually with 7 years but less than 12 years of service.
30 days accrued annually with 12 years of service or more.
After 6 month benefited probationary period.
Retirement Plan
1 year waiting period for enrollment.
20% vested with 1 year but less than 2 years of participation.
40% vested with 2 years but less than 3 years of participation.
60% vested with 3 years but less than 4 years of participation.
80% vested with 4 years but less than 5 years of participation.
100% vested with 5 years of participation or more.
Hospital contributes 7% of annual salary.
Employee may contribute up to 60% of annual salary, pretax contributions.
Employee selects from 12 investment options.
Educational Leave
5 days per fiscal year.
$500 free.
$500 matching dollar per dollar.
Mental Health Workers /Case Managers /Assistant Case Managers /Employment
Support Specialists have a one year waiting period.
Employees with less than one year of service will earn $125.00 quarterly,
cumulative per calendar quarter, per fiscal year.
Paid Holidays
6 paid holidays per year.
Malpractice Insurance
Malpractice insurance provided to all clinical staff for clients treated
within the Wabash Valley Hospital system.
