Voluntary Medical Plan 

United HealthCare

Traditional-PPO Plan

$2,000 individual deductible / $4,000 family deductible per calendar year

$20 office Copay for services by primary/$30 Copay for specialty care physician

$0 IP Copay / $250 ER Copay / $50 UC Copay

Pharmacy $10/$35/$60

 80% Coinsurance; $6,600 Individual Out-of Pocket, $13,200 Family Out-of-Pocket

If an employee’s spouse has other medical coverage available to them,

they are not eligible to enroll in Wabash Valley Alliance plans

Cost to employee - $48.85 per pay period (every 2 weeks)

Full time employee plus one dependent - $116.76 per pay period (every 2 weeks)

Full time employee two or more dependents - $141.16 per pay period (every 2 weeks)

Health Savings Plan (HSA)

$2,600 individual deductible / $5,200 family deductible per calendar year

Employer HSA Contribution: $48 per pay period for Employee Only

80% Coinsurance once deductible met

$6,350 Individual Out-of Pocket, $12,700 Family Out-of-Pocket

Cost to employee - $39.81 per pay period (every 2 weeks)

Full time employee plus one dependent - $95.16 per pay period (every 2 weeks)

Full time employee two or more dependents - $115.04 per pay period (every 2 weeks)

30 day waiting period for enrollment in PPO or HSA

 

Voluntary Dental Plan

United HealthCare

$50 per person/$150 per family deductible per annual calendar year

High Plan, $1,000 Annual Maximum, $1000 Lifetime Ortho Maximum, 50% coinsurance;

Low Plan, $750 Annual Maximum, No Ortho Benefit, 50% coinsurance

Cost to full time employee - $12.22 (High Plan), $8.20 (Low Plan)

Full time employee plus spouse- $24.34 (High Plan), $16.35 (Low Plan)

Full time employee plus child (ren) - $35.04 (High Plan), $23.53 (Low Plan)

Full time employee plus family - $47.16 (High Plan), $31.65 (Low Plan)

30 day waiting period for enrollment – May not drop during year

 

Voluntary Vision Plan

Cost to full time employee - $3.06

Full time employee plus spouse - $5.65

Group Life Insurance Plan

Life Insurance coverage available in amounts of 200% of your annual earnings,

rounded to the nearest $1, up to a maximum of $350,000

Accidental Death and Dismemberment Insurance benefit equal the Life amount

AD&D Higher Education Benefit and an Automobile Accident Benefit

Benefits are reduced starting at age 70

Accelerated Benefit that pays up to 80% of the Life benefit to a maximum of $350,000 in the

event of a life-threatening medical condition where there is a life expectancy of 12 months or less

60 day minimum service requirement

 

Group Short-Term Disability Insurance

Weekly benefits are equal to 50% of weekly pre-disability pay,

to a maximum of $1,800 per week; minimum benefit of $25

Benefits begin on the 8th day of disability due to accident and the

8th day of disability due to sickness or pregnancy

25 week benefit duration.

60 day minimum service requirement

 

Group Long-Term Disability Insurance

Monthly benefits are equal to 50% of monthly pre-disability pay,

to a maximum of $8,000 per month; minimum benefit of $1000

6 month qualifying period

 

Annual Leave

15 days accrued annually with less than 2 years of service

20 days accrued annually with 2 years but less than 7 consecutive years of service

25 days accrued annually with 7 years but less than 12 consecutive years of service

30 days accrued annually with 12 years but less than 20 consecutive years of service

35 days accrued annually with 20 consecutive years of service or more

After 6 month benefited probationary period

 

Retirement Plan- 401K

1 year waiting period for enrollment

20% vested with 1 year but less than 2 years of participation

40% vested with 2 years but less than 3 years of participation

60% vested with 3 years but less than 4 years of participation

80% vested with 4 years but less than 5 years of participation

100% vested with 5 years of participation or more

Employer contributes 7% of annual salary

Employee may contribute up to 100% of annual salary, pretax contributions

Employee selects from 16 investment options

 

Educational Leave

5 days per fiscal year (July 1st to June 30th)

$500 provided

$500 matching dollar per dollar

 Mental Health Workers/Case Managers/Assistant Case Managers/Employment

Support Specialists have a one year waiting period

MPT with less than one year of service will earn $125.00 quarterly,

cumulative per calendar quarter, per fiscal year

Paid Holidays

6 paid holidays per calendar year

 

Malpractice Insurance

Malpractice insurance provided to all clinical staff for clients treated

within the Wabash Valley Alliance system

 

Assurant Voluntary Life – Aflac Accident, Critical Illness & Hospital Supplemental Insurance Available
